Missing Episodes and Incomplete Series

Does anybody know why some series aren't uploaded in their entirety? How come we can't find certain episodes of a series online? How does it work, anyway? Not all of us are in it for popularity. Some of us are just in it for uploading screenshots on wikis. And it's best to include as many accurate facts as we can. Who exactly does the uploading, anyway?
